Dispute Resolution Form

This form is designed to be used by employees that are covered by the Colorado Partnership for Quality Jobs and Services Act and the associated Partnership Agreement to submit a Partnership Agreement Dispute.

When a covered employee elects to file a dispute under the Partnership Agreement rather than appealing to the State Personnel Board, the covered employee is required to sign a written acknowledgement that they are waiving the right to pursue an appeal through the State Personnel Board or State Personnel Director. This waiver form needs to be completed by the covered employee and submitted along with the Partnership Agreement Dispute Resolution Form.
